Case Manager – Registered Nurse

This is a fully remote position, open to applicants in Texas.

📋 Description

• Conduct telephonic assessments and occasional in-person evaluations to understand member needs.

• Create and oversee personalized care plans aimed at enhancing health and wellness.

• Facilitate care coordination across various programs, providers, and resources.

• Utilize clinical judgment to identify risks and eliminate barriers to care.

• Engage members in meaningful discussions to support their health objectives.

• Collaborate with a multidisciplinary team to maximize outcomes.

⛳️ Requirements

• Must possess an active, unrestricted multistate RN license in your state of residence.

• A minimum of 3 years of clinical nursing experience is required.

• Proficient computer skills, including Microsoft Office and web-based systems, are essential.

• Ability to work Monday to Friday from 8:00am to 5:00pm CST.

• Occasional rotation to a schedule of 10:00am to 7:00pm CST may be required.

• An Associate's Degree in Nursing is mandatory.

• Experience in case management within an integrated model is preferred.

• Bilingual proficiency in Spanish and English is preferred.

• A Bachelor's degree in Science & Nursing is preferred.
