Case Manager, Registered Nurse

This is a fully remote position, open to applicants in Iowa.

📋 Description

• Facilitate the provision of appropriate benefits and healthcare information to assess benefit eligibility.

• Encourage participation in wellness initiatives.

• Create, execute, and enhance health strategies, policies, and programs.

• Ensure the effective delivery of benefits and foster member wellness.

• Assist with timely and successful return-to-work processes.

• Address issues related to network management, clinical coverage, and associated policies and programs.

• Perform occasional in-person case management activities within a specified geographic area as directed by leadership or business requirements.

⛳️ Requirements

• Active Iowa RN or Compact License is required.

• A minimum of 3-5 years of clinical practical experience is preferred, particularly in diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac care involving Medicare members.

• Ability to travel occasionally within a designated geographic area for in-person case management activities.

• At least 2-3 years of experience in case management, discharge planning, and/or home health care coordination is preferred.

• Strong analytical and problem-solving abilities.

• Excellent communication, organizational, and interpersonal skills.

• Capability to work independently, potentially from home.

• Proficiency in MS Word, Excel, Outlook, PowerPoint, and proprietary software.

• Competent computer skills, including navigating various systems and proficient typing.

• Willingness to obtain multistate RN licenses if necessary; support will be provided by the company.

• Bilingual candidates are preferred.

• Certification as a Case Manager is preferred.

• An Associate Degree in Nursing is required.

• Registered Nurse licensure in Iowa is mandatory.

• Compact licensure is preferred.

• Additional national professional certifications (such as CRC, CDMS, CRRN, COHN, or CCM) are preferred but not mandatory.
