This is a fully remote position, open to applicants in Alaska, +26 more states.
• Engage with target members experiencing medical, mental health, and/or substance use disorders to foster a relationship that encourages open and constructive discussions on sensitive issues.
• Conduct comprehensive holistic health assessments to gain insight into a member's unique circumstances and needs, while identifying social determinants of health (SDOH) and cultural factors that may affect potential lifestyle modifications.
• Establish a collaborative environment where a case manager and member, along with pertinent caregivers and providers, can create a member-focused case management plan that sets objectives, addresses potential obstacles, and aims for enhanced health outcomes.
• Monitor, assess, and revise the effectiveness of the member's case management plan to ensure ongoing progress toward established goals and that current objectives remain relevant.
• Facilitate communication and coordination among the healthcare team to optimize the delivery of appropriate care.
• Implement interventions aimed at eliminating barriers, encouraging positive behavior changes through motivational interviewing techniques, and stabilizing the members’ natural support systems by ensuring caregiver needs are recognized and addressed.
• Maintain concise, accurate, and timely documentation that complies with accreditation, contractual, and legal standards.
• Active WA State or Compact License in one of the following: Nursing (current registration as a nurse, e.g., RN, ARNP), Behavioral Health (valid licensure, e.g., LMFT, LICSW, LMHC), or Psychology (board certification).
• Minimum of 3 years of experience in direct patient or clinical care post-licensure.
• Internet access through any Internet Service Provider (ISP) with a required minimum download speed of 75 Mbps (megabits per second).
• Over 1 year of prior experience as a Case Manager.
• Certified Case Manager (CCM) certification.
• Chemical Dependency Professional (CDP) certification.
• Chronic Care Professional certification (CCP).
• LEAN training.
• Advanced skills in Motivational Interviewing.
