Case Manager

This is a fully remote position, open to applicants in Alaska, +3 more states.

📋 Description

• Serve as a bridge between patients, primary care doctors, and various healthcare professionals.

• Evaluate, plan, implement, coordinate, oversee, and assess patient options.

⛳️ Requirements

• Associate's Degree in Nursing is required.

• A valid RN License is mandatory.

• A minimum of three years of clinical care experience is essential.

• Active CCM Certification or the capacity to obtain it within 18 months.

• Proven problem-solving abilities.

• Strong organizational and time management skills.

• Capability to work autonomously and complete assignments promptly.

• Exceptional customer service abilities.

• Proficient in documenting system notes while interacting with callers.

• Effective communication skills, both verbal and written.

• Proficient in MS Word, Excel, and Outlook.
