Case Management Specialist

This is a fully remote position, open to applicants in Virginia.

📋 Description

• The Case Management Specialist is responsible for coordinating and executing administrative tasks to assist the Case Management department.

• Engages in the annual and ongoing budgeting processes.

• Organizes services for designated patient groups, which include Home Health, Durable Medical Equipment, Ambulance, Housing, Skilled Nursing Facilities, and others, to ensure that suitable continuing care services are established for patient transitions.

• Effectively communicates and documents precise clinical and financial information in both verbal and written formats.

• Provides and records the delivery of regulatory documents, such as the required Observation letter and Medicare IMM notifications.

• Ensures timely service arrangements to fulfill customer needs.

• Quickly addresses and resolves provider and payor concerns with the assistance of Case Management staff.

• Monitors and tracks accounting for budgets, capital requests, travel expenses, and business costs.

⛳️ Requirements

• Education: High School diploma or GED.

• Experience: Minimum of 2 years in a relevant field.

• Preferred Qualifications: Administrative or clerical experience in the healthcare sector.

• Experience in case management within home health, rehabilitation, durable medical equipment, or skilled nursing environments.
