Case Management Coordinator

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Perform routine care coordination, support, and education utilizing care management resources.

• Facilitate favorable healthcare outcomes for waiver/LTSS members.

• Execute projects, programs, and processes related to Case Management.

• Manage a full caseload of waiver members.

• Travel 50–75% of the time for face-to-face meetings with members.

• Conduct in-person quarterly visits with members.

• Assess the needs, eligibility, benefit plans, and available programs/services for referred members.

• Identify high-risk factors and service requirements, making referrals to clinical case management or crisis intervention as necessary.

• Coordinate, implement, and oversee care plan activities.

• Consult with case managers, supervisors, Medical Directors, and other health programs to address barriers.

• Present cases during multidisciplinary case conferences.

• Identify and report quality-of-care issues.

• Negotiate suitable options and services for members.

• Utilize influencing and motivational interviewing techniques to engage members and encourage lifestyle/behavior changes.

• Offer coaching, information, and support for independent medical and healthy lifestyle decisions.

• Assist members in engaging in healthcare decision-making alongside providers.

• Implement case management and quality management processes in alignment with regulatory, accreditation, and company standards.

⛳️ Requirements

• Must reside in Illinois, specifically in Fayette County or surrounding areas.

• Willing and able to travel up to 75% of the time for in-person meetings with members.

• Reliable transportation is required, eligible for mileage reimbursement in accordance with company policy.

• Minimum of 2 years of experience in the Behavioral Health, Social Services, or Human Services field.

• At least two (2) years of case management experience is required.

• Valid driver’s license is necessary, preferably issued in IL.

• Bachelor's degree or a non-licensed individual with a master’s degree in a Behavioral Health or Human Services field is required.

• Experience in case management and discharge planning is preferred.

• Managed Care experience is preferred.

• Proficiency in Microsoft Office is preferred.
