This is a fully remote position, open to applicants in Pennsylvania.
• Coordinate appointments, schedules, aftercare planning, and meetings related to treatment for clients.
• Monitor the progress of clients and work in collaboration with the treatment team to address any emerging needs.
• Create, facilitate, and oversee individualized care plans.
• Conduct orientations for clients and their families in partnership with clinical staff.
• Provide continuous case management support to both clients and their families.
• Connect clients with community resources, healthcare providers, transportation services, and aftercare support.
• Ensure clients have access to a comprehensive range of both traditional and non-traditional services.
• Assist with coordinating referrals and maintaining communication with providers throughout the treatment process.
• Support recovery engagement initiatives by utilizing assessment data and addressing the individualized needs of clients.
• Verify the completion of admission documents, consents, and necessary records.
• Maintain accurate client records and documentation in alignment with federal, state, and accreditation standards.
• Safeguard confidential client information and uphold HIPAA compliance.
• Communicate consistently with referral sources, community providers, family members, legal representatives, employers, and internal treatment teams.
• Participate in treatment team meetings and case reviews.
• Collaborate with outreach and marketing teams to enhance relationships with community providers.
• Master's degree from an accredited college or university in Social Work, Psychology, Counseling, Nursing, Chemical Dependency Studies, or a related behavioral health field.
• Addiction counseling certification is preferred.
• Preferred experience or capability in Level of Care Assessment.
• Background in behavioral health, mental health, substance use disorder treatment, recovery support services, or case management is necessary.
• Experience in facilitating individual, family, or process groups is preferred.
• Preferred experience in treatment planning and care coordination.
• Strong verbal and written communication skills are essential.
• Exceptional abilities in relationship-building and client engagement.
• Proficient clinical assessment and critical thinking skills.
• Skills in crisis intervention and problem-solving are required.
• Knowledge of behavioral health, addiction treatment, and community resources is important.
• Excellent organizational and documentation skills are necessary.
• Ability to manage multiple priorities in a remote setting.
• Understanding of state, federal, and accreditation requirements for behavioral healthcare services.
