Care Transitions Care Manager – North-Central

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Deliver in-reach and transition care management to individuals living in institutional, congregate, or facility-based environments, such as hospitals and inpatient units.

• Inform members, families, and guardians about integrated community-based care, available services, and informal/community resources.

• Assist members and guardians in accessing health, behavioral, and social service resources.

• Provide comprehensive care management for individuals with behavioral health, physical health, co-occurring, co-morbid, or multi-morbid conditions.

• Create care plans that are based on national evidence-based and informed standards for holistic care.

• Direct and oversee providers concerning social determinants, medical, and various member needs.

• Recognize, record, and strive to resolve obstacles to moving to more integrated community settings.

• Engage members in state psychiatric hospitals and ACH settings regarding permanent supportive housing, supports, services, and community resources.

• Offer opportunities for members and guardians to connect with peers living, working, and receiving services in community settings.

• Complete necessary documentation, paperwork, and tasks within software platforms within designated timelines.

• Collaborate with an interdisciplinary team addressing Olmstead Settlement and Tailored Plan functions.

• Evaluate and oversee member health and safety, including during emergencies or weather warnings.

• Travel within the designated catchment area as needed.

⛳️ Requirements

• Must hold QP status.

• Fully licensed by the North Carolina governing board that regulates Human Services professions; or fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing.

• RN pathway necessitates four years of full-time experience with the served MH/DD/SA population.

• A Master’s degree in a human service field along with one year of full-time pre- or post-graduate MH/DD/SA experience, or one year of supervised alcoholism and drug abuse counseling experience.

• A Bachelor’s degree in a human service field plus two years of full-time pre- or post-bachelor’s MH/DD/SA experience, or two years of supervised alcoholism and drug abuse counseling experience.

• A Bachelor’s degree in a non-human-services field plus four years of full-time pre- or post-bachelor’s MH/DD/SA experience, or four years of supervised alcoholism and drug abuse counseling experience.

• Must possess a valid driver’s license.

• Must reside within Trillium’s North Central Region: Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, or Wilson County, North Carolina.

• Must be able to travel within the catchment area as required.

• All applicants must pass a drug test as a condition of employment.
