This is a fully remote position, open to applicants in North Carolina.
• Deliver in-reach and transition care management to individuals in institutional, congregate, or facility-based environments, such as ACHs, hospitals, and inpatient units.
• Inform members, families, and guardians about integrated community-based care options, available services, and informal/community resources.
• Assist members and guardians in accessing physical, behavioral, and social service resources.
• Create care plans based on national evidence-based and informed standards for comprehensive whole-person care.
• Offer guidance and oversight to providers regarding social determinants, medical issues, and other member needs.
• Complete all necessary documentation, paperwork, and tasks in Trillium’s software platform within specified timeframes.
• Collaborate with an interdisciplinary team to address the Olmstead Settlement and Tailored Plan functions.
• Evaluate and monitor member health and safety, particularly during emergencies or severe weather advisories.
• Deliver complex care management that adheres to NCQA and other regulatory/accreditation standards.
• Identify, document, and strive to overcome obstacles to relocation to integrated community settings.
• Engage with members in State Psychiatric Hospitals and ACHs concerning permanent supportive housing, services, and community resources.
• Travel within the catchment area as needed.
• Must fulfill QP status.
• Fully licensed by the NC governing board regulating Human Services professions; or be a fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing.
• Licensed RN alternative requires four years of full-time experience with the MH/DD/SA population.
• Master’s degree in a human service field plus one year of full-time MH/DD/SA experience, or one year of supervised alcoholism and drug abuse counseling experience.
• Bachelor’s degree in a human service field plus two years of full-time MH/DD/SA experience, or two years of supervised alcoholism and drug abuse counseling experience.
• Bachelor’s degree in a non-human-services field plus four years of full-time MH/DD/SA experience, or four years of supervised alcoholism and drug abuse counseling experience.
• Must possess a valid driver’s license.
• Must reside within Trillium’s Northern Region: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, or Warren counties.
• Must be able to travel within the catchment area as needed.
• All candidates must successfully complete a drug test as a condition of employment.
