This is a fully remote position, open to applicants in Oregon.
• Oversee physiological indicators such as blood pressure, weight, and blood glucose with clinical appropriateness.
• Achieve team objectives and standards based on specified metrics.
• Make a significant contribution to long-term patient engagement in RPM programs.
• Deliver education and coaching on preventive health and disease management.
• Conduct monthly wellness calls with designated patients.
• Facilitate collaborative wellness discussions with patients to establish health goals set by their Care Team.
• Manage patient communications and alerts.
• Guide patients to their treating physician for routine inquiries.
• Attain patient engagement program targets.
• Adhere to proper escalation procedures for any urgent care requirements.
• Unrestricted RN/LVN/LPN license in a compact state and Oregon.
• Availability to work in the PST zone from 8:00 a.m. to 5:00 p.m.
• Proficiency in Spanish is a plus.
• 1-3 years of clinical experience, patient management, or disease management preferred.
• Familiarity with various provider practices and workflows.
• Quick learners.
• Capability to work independently with minimal supervision.
• Experience with Medicare patients.
• Background in performing virtual visits with patients and managing care via telephone.
• Enthusiasm for professional leadership development opportunities within a growing organization.
• Interest in participating in a new, dynamic clinical program and playing a key role in the establishment of processes and best practices for RPM patients.
• Experience in Motivational Interviewing/Health Behavior Change is a plus.
• Health Coach certification is a plus.
