
Care Manager – Social Worker

This is a fully remote position, open to applicants in United States.
• Conduct in-home and telehealth care management visits to evaluate and establish social and behavioral conditions.
• Collaborate closely with the Care Team to ensure teamwork and achieve optimal patient outcomes.
• Evaluate social determinants of health needs and formulate a strategy to address them.
• Identify, assess, and foster relationships with local Community-Based Organizations.
• Provide education to patients on suitable resources, assist with completing referrals, and follow up for closure outcomes.
• Act as a subject matter expert on social determinants for other Care Team members.
• Conduct assessments related to behavioral, environmental, and social support.
• Facilitate individual, family, and group education on managing chronic illnesses.
• Involve family and social support groups in patient education and care.
• Evaluate patients and refer them to behavioral health specialists if diagnosis and treatment are necessary.
• Assist patients in understanding, accepting, and adhering to medical and lifestyle recommendations.
• Review and document patient updates and progress within the care management platform.
• Currently licensed as an LCSW or LMSW in the specified state.
• Master’s degree in social work and successful completion of the ASWB masters or clinical exam.
• Basic Life Support (BLS) certification is mandatory for this position.
• Self-motivated individual capable of working independently with minimal supervision.
• Ability to demonstrate empathy and quickly establish relationships with patients and local Community-Based Organizations.
• Preferred 2+ years of relevant experience in care management and/or working with chronic illnesses.
• Exceptional verbal communication skills, both in person and over the phone.
• Proficient in Microsoft Office and familiar with mobile phone and web-based applications.
