This is a fully remote position, open to applicants in Missouri.
β’ Develops, evaluates, and implements intricate care management activities primarily for members with physical needs.
β’ Ensures high-quality, cost-effective healthcare outcomes by creating personalized care plans and providing education to members and their families.
β’ Assesses member needs and identifies barriers to accessing appropriate care.
β’ Formulates ongoing care/service plans and collaborates with healthcare providers.
β’ Detects issues and barriers to care, implementing suitable care management interventions.
β’ Facilitates coordination between the member and/or their family/caregivers and the care provider team.
β’ Conducts continuous follow-up and monitoring of member status.
β’ Offers resource support to members and care managers regarding local resources.
β’ Collects, documents, and maintains comprehensive member information and care management activities.
β’ Provides and/or organizes educational resources for members and their families/caregivers.
β’ A Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing is required.
β’ 2 to 4 years of relevant experience is necessary.
β’ RN - Registered Nurse - State Licensure and/or Compact State Licensure is mandatory.
β’ Experience in Foster Care is preferred.
β’ Familiarity with ages 13-26 is required.
β’ Knowledge of Physical Health and an understanding of Behavioral Health is essential.
