Care Manager, RN – Foster Care

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Develops, evaluates, and implements intricate care management activities primarily for members with physical needs.

• Ensures high-quality, cost-effective healthcare outcomes by creating personalized care plans and providing education to members and their families.

• Assesses member needs and identifies barriers to accessing appropriate care.

• Formulates ongoing care/service plans and collaborates with healthcare providers.

• Detects issues and barriers to care, implementing suitable care management interventions.

• Facilitates coordination between the member and/or their family/caregivers and the care provider team.

• Conducts continuous follow-up and monitoring of member status.

• Offers resource support to members and care managers regarding local resources.

• Collects, documents, and maintains comprehensive member information and care management activities.

• Provides and/or organizes educational resources for members and their families/caregivers.

⛳️ Requirements

• A Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing is required.

• 2 to 4 years of relevant experience is necessary.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is mandatory.

• Experience in Foster Care is preferred.

• Familiarity with ages 13-26 is required.

• Knowledge of Physical Health and an understanding of Behavioral Health is essential.
