This is a fully remote position, open to applicants in North Carolina.

đź“‹ Description

• Conduct thorough assessments or Care Needs Screening during enrollment, annually, or when conditions change.

• Create Plans of Care based on the assessments completed.

• Delegate interventions/plans of care to the relevant Alliance Care Management team members to address identified member needs, for monitoring, and/or service engagement activities.

• Refer cases to the CCM when there are physical health or behavioral health needs that suggest medical and/or pharmaceutical complexity.

• Assist individuals or their legally responsible persons in selecting service providers while ensuring an impartial process.

• Continuously assess the suitability of services and ensure the execution of the plan of care through information collection and evaluations at specified contact frequencies based on risk stratification.

• Employ person-centered planning, motivational interviewing, and a historical review of assessments in JIVA to gather information and determine necessary supports for the individual.

• Collaborate proactively with members, legally responsible persons, care teams, service providers, and identified supports to develop a plan that accurately represents the individual’s needs and desired life goals, adhering to best practices and utilizing the permanent supportive housing model.

• Offer education and support to individuals and legally responsible persons regarding their rights, the grievance and appeals process, available service options, providers suited to their needs, and payer requirements that may influence service access and maintenance.


⛳️ Requirements

• A Bachelor’s degree from an accredited college or university in a Human Services field and two (2) years of post-bachelor’s degree mental health/developmental disabilities/substance abuse experience with the population served.

• A Bachelor’s degree from an accredited college or university in a Non-Human Services field and four (4) years of post-bachelor’s degree mental health/developmental disabilities/substance abuse experience with the population served.

• A Master’s Degree from an accredited college or university in a Human Services field and one (1) year of post-graduate degree mental health/developmental disabilities/substance abuse experience with the population served.

• Fully or Provisionally Licensed in the State of North Carolina as a LCSW, LCMHC, LPA, or LMFT.

• A Licensed Registered Nurse (RN) in the State of North Carolina with four (4) years of mental health/developmental disabilities/substance abuse experience with the population served.

• Preferred: NACCM, NADD-Specialist and/or CBIS Certification.

• Knowledge, Skills, & Abilities in Person-Centered Thinking/Planning.

• Familiarity with utilizing assessments to formulate plans of care.

• Understanding of the Diagnostic and Statistical Manual of Mental Disorders.

• Knowledge of the LOC process, SIS for IDD, and FASN assessment for TBI.

• Familiarity with Medicaid Tailored Plan, Medicaid Direct, enhanced MHSUD, and waiver benefits plans.

• Knowledge and proficiency in the application of Motivational Interviewing.

• Proficient in Microsoft Office applications (such as Word, Excel, Outlook, etc.).
