This is a fully remote position, open to applicants in New Jersey.
• Design, evaluate, and implement advanced care management initiatives for members primarily dealing with mental and behavioral health issues.
• Develop individualized care plans and offer education for both members and their families regarding mental health and substance use disorders.
• Assess member needs through phone calls or in-home visits and recommend or facilitate appropriate care and service plans.
• Perform telephonic, digital, home, and other outreach visits to evaluate member needs.
• Create ongoing care plans for high-acuity members, identifying suitable providers, specialists, and community resources.
• Collaborate with members, families, caregivers, community resources, and care provider teams to ensure accessibility of services.
• Monitor care plans, member conditions, treatment side effects, complications, and clinical symptoms.
• Facilitate coordination of care with providers and specialists to guarantee timely access to necessary services.
• Gather, document, and retain member information and care management activities to ensure compliance with regulatory standards.
• Educate members and families on procedures, provider instructions, treatment alternatives, referrals, and healthcare benefits.
• Provide leadership insights on areas for enhancing care quality and delivery.
• Execute other assigned tasks while adhering to policies and standards.
• Applicants must reside in Passaic or Sussex Counties, New Jersey, or a nearby bordering county.
• An active New Jersey license is required.
• A Master's degree in Behavioral Health or Social Work, or a degree from an accredited nursing school is necessary.
• 2–4 years of relevant experience is essential.
• Licensed Master's Behavioral Health Professional credentials, such as LCSW, LMSW, LMFT, LMHC, or LPC, or RN as per state contract requirements, with behavioral health experience is mandatory.
• For NJ Health Plan: A Master's degree in Behavioral Health or Social Work along with a Licensed Associate Counselor (LAC) or Licensed Social Worker (LSW) is required.
• Availability from Monday to Friday, 9:00 AM to 5:00 PM is needed.
• Capability to conduct phone, digital, home, and other site visits is essential.
• Knowledge of mental health, substance use disorders, care coordination, healthcare benefits, and social determinants of health is expected.
• Proficiency in documenting care management activities and ensuring compliance with state, federal, and third-party payer regulations is required.
