Care Manager

This is a fully remote position, open to applicants in Texas.

📋 Description

• In the position of Care Manager, you will be essential in researching, evaluating, and planning interventions for clients dealing with social, emotional, economic, and environmental issues.

• You will play a key role in the development and enhancement of our care management program.

• This role involves assisting policyholders of long-term care insurance by facilitating their access to community resources, services, and support.

• Utilizing assessment findings, screenings, and your comprehensive knowledge of community-based resources, you will aid policyholders in reaching their care journey objectives.

⛳️ Requirements

• A robust background in geriatric case management or coordination experience is required.

• Experience in one or more of the following fields is highly desirable: Social Services, Care Coordination, Healthcare Navigation, Public Health, Community Organizing, or Volunteer Work.

• A minimum of 1 year of relevant experience in customer service or a similar role is preferred.

• Experience in a High Volume Call Center is an advantage!

• A Bachelor’s degree in Psychology or Social Work is preferred.

• Applicants must be 18 years of age or older.
