This is a fully remote position, open to applicants in Massachusetts.
• Conduct telephonic outreach to members and/or engage in face-to-face interactions, employing essential motivational interviewing techniques to encourage program enrollment.
• Carry out departmental assessments and assess members holistically to determine their needs, health objectives, and obstacles to wellness.
• Create a personalized care plan for each member, implement specific interventions from the care manager, and modify the care plan as necessary.
• Ensure documentation is completed in the appropriate platform in accordance with departmental policies and regulatory standards.
• Deliver targeted health education, proactive condition management strategies, and maintain communication with key providers and vendors involved in the member’s care.
• Work in collaboration with the member/caregiver and the facility care team to ensure a secure transition to the next level of care.
• Collaborate and connect with the interdisciplinary care team to enhance member outcomes.
• Pursue professional growth and development through self-directed learning initiatives.
• Current unrestricted Registered Nurse license in the state of residence.
• May need to obtain additional state licensure in locations where Point32Health operates.
• Comprehend and adhere to the regulations set forth in state-specific Nurse Practice Act(s) applicable to Point32Health operations.
• National certification in Case Management is preferred.
• Bachelor’s degree or equivalent relevant experience is required.
• A minimum of 5 years of pertinent clinical experience.
• Proficiency in a second language is a plus.
• Experience in specialized areas is advantageous.
• Competence in Microsoft applications, technical concepts, and principles; familiarity with computer software applications.
• Ability to collaborate effectively as a team member across various levels within the organization.
