Care Manager

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Address the requirements of the served population by assessing, planning, implementing, coordinating, monitoring, and evaluating the necessary options and services, utilizing communication and available resources to enhance quality and cost-effective health outcomes.

• Deliver efficient Care Management services in alignment with case management standards of practice for enrolled populations.

• Conduct comprehensive member assessments that consider the individual as a whole, encompassing medical, biopsychosocial, behavioral, spiritual, and cultural needs.

• Collaborate with multi-disciplinary team members to promote the successful achievement of desired treatment outcomes.

• Establish and maintain collaborative relationships with community provider agencies that support quality care and cost-effective healthcare utilization.

⛳️ Requirements

• Registered Nurse (RN)

• Graduation from an accredited nursing school; BSN preferred

• Active, unrestricted RN license for practice in North Carolina

• Minimum of 2 years nursing experience; 1 year in care management or community-based nursing preferred

• CCM certification preferred; must be obtained within 1 year of eligibility according to CCM requirements

• At least 2 years of social work experience; 1 year in case management or community-based social work preferred

• Master’s degree from an accredited school of social work

• Active NC license as a Licensed Clinical Social Worker (LCSW)
