Care Coordinator, Spanish & English Proficiency

This is a fully remote position, open to applicants in Colombia.

📋 Description

• Manage a high volume of incoming calls (80+ weekly), acting as the main point of contact for clients, families, caregivers, referral sources, and healthcare professionals.

• Coordinate patient care, referrals, and ensure timely scheduling of both initial and follow-up appointments.

• Review physicians' notes, upload prescriptions, and verify that care plans are up-to-date and accurate.

• Schedule, reschedule, and oversee care assignments across various locations using cloud-based scheduling tools.

• Utilize scheduling and care management software to log updates, monitor staff assignments, and manage caseloads effectively.

• Confirm patient attendance, track treatment progress, and follow up with providers to ensure continuity of care.

• Keep precise client records, document all communications, and revise care plans as necessary.

• Conduct follow-ups with providers and handle escalations in accordance with established Standard Operating Procedures (SOPs).

• Communicate effectively with field staff, clients, and internal departments to ensure adequate coverage and smooth care operations.

⛳️ Requirements

• Experience in care coordination (preferably in home care or a comparable healthcare coordination role).

• Familiarity with home care scheduling software (e.g., HHAeXchange, Epic, Athenahealth, or similar EHR systems).

• Strong communication skills—ability to manage a high volume of calls while building rapport over the phone.

• Understanding of industry regulations and compliance requirements (e.g., HIPAA, state home care regulations).

• Excellent organizational and multitasking skills, with a keen attention to detail.

• Proficiency in Microsoft Office (Word, Excel, Outlook) and openness to learning care management or scheduling systems.

• Compassionate, service-oriented attitude and capability to remain calm under pressure.
