Care Coordinator, LPN, LVN

This is a fully remote position, open to applicants in Nebraska.

📋 Description

• Oversee patient census with a focus on resolution to address gaps in both clinical and non-clinical patient care.

• Recognize and coordinate community resources that would enhance patient care.

• Deliver patient education and promote health literacy regarding chronic condition management.

• Conduct medication management, which includes identifying potential medication issues, reconciliation, adherence, and coordinating refills.

• Help ensure prompt delivery of services to patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patients' inquiries and foster an open dialogue to comprehend their needs.

• Support/manage referrals and the scheduling of appointments.

⛳️ Requirements

• Active Compact/Multi-State license (LPN) (LVN)

• Technical proficiency in Microsoft Office Suite

• Outstanding written and verbal communication abilities

• Experience with Epic (Plusses)

• Bilingual capabilities (Plusses)

• Additional Single State licensures (Plusses)
