
Care Coordinator – Home Care Operations

This is a fully remote position, open to applicants in California.
• Act as the main point of contact for a designated caseload of Continuous Care clients and their families.
• Develop and manage caregiver schedules for ongoing cases, ensuring consistency and continuity wherever feasible.
• Address caregiver call-outs, schedule interruptions, and urgent coverage requirements in real time.
• Swiftly arrange replacement coverage while ensuring a positive client experience.
• Pair caregivers with clients based on compatibility, skill set, availability, and family preferences.
• Conduct onboarding calls with clients and adequately prepare caregivers for new assignments.
• Provide guidance to caregivers regarding client-specific expectations, care plans, and performance issues.
• Handle client concerns, complaints, and service recovery situations with professionalism and promptness.
• Proactively engage with families to detect changes in needs before they escalate into significant issues.
• Collaborate closely with Recruiting, HR, Sales, and Leadership to ensure that staffing pipelines meet client demand.
• Review caregiver visit notes to identify trends, gaps, or areas for enhancement.
• Accurately and consistently document all communication, schedule modifications, client updates, and action items.
• Exercise critical thinking and make independent judgment calls while escalating concerns as necessary.
• Minimum of 2 years of experience in care coordination, staffing, scheduling, client services, or operations within a licensed home care agency.
• Direct experience in staffing ongoing caregiving cases and managing caregiver schedules.
• Experience in managing caregiver call-outs and urgent service recovery scenarios.
• Strong understanding of caregiver-client matching principles.
• Excellent communication skills via phone, email, and internal systems.
• Capability to work independently in a fully remote setting.
• Bachelor’s degree preferred but not mandatory.
