This is a fully remote position, open to applicants in New Jersey.
• Implement individualized care plans for patients facing serious, high-risk health conditions. Assist in the monitoring and modification of disease-specific plans, particularly when frequent changes in medication or treatment are necessary.
• Serve as the connection between the patient and the billing provider. Facilitate care coordination by managing provider availability, rescheduling appointments, and offering support after hospital discharge.
• Perform person-centered evaluations to grasp the patient’s social and cultural background. Identify unmet social needs—such as food insecurity or housing instability—that may affect the management of their serious illness and create targeted action plans to address these issues.
• Aid patients and their caregivers in navigating the healthcare system in New Jersey. Educate patients on self-advocacy skills to enhance their communication with specialists and help coordinate transportation or access to telehealth services.
• Support the development of patient communication templates and automation processes. Collaborate with the technical team to establish workflows, troubleshoot technical issues, and accurately update patient records in the Case Management System (CMS).
• Direct patients to supportive services and community resources. Build and maintain partnerships with local organizations in New Jersey to highlight essential resources (such as legal aid and food assistance) to our members.
• Utilize motivational interviewing and capacity-building techniques to assist patients in managing the stress of chronic illness. Provide customized education to enhance treatment adherence and offer emotional support to families and caregivers.
• Keep meticulous records of all interactions, including time spent and specific activities undertaken. Ensure that all documentation complies with the standards necessary for Medicare clinical and social integration services.
• Preferred training or certification as a Community Health Worker (CHW). Equivalent experience in community health, social services, or a related field will also be considered.
• Compliance: Willingness to complete and maintain all CMS-required training and state-specific standards, including skills in interpersonal relations, service coordination, and professional ethics.
• Experience: A minimum of 3 years of relevant experience in community outreach, facilitation, or healthcare. Experience in managing high-risk chronic conditions or complex care is highly advantageous.
• Technical Proficiency: Strong computer skills and the ability to navigate web-based and app-based systems effectively.
• Cultural Awareness: A profound understanding of the socio-economic and public health challenges faced by marginalized populations in New Jersey.
• Communication Skills: Excellent written and verbal communication abilities, with a personable demeanor and the capacity to inspire and influence diverse individuals.
• Logistics: A high school diploma/GED is required (a college degree is preferred). Must possess a valid driver’s license, reliable transportation, and a flexible schedule for occasional work outside regular business hours.
