
Bilingual Spanish/English Primary Care APP (NP/PA) – Telehealth

This is a fully remote position, open to applicants in Arizona, +1 more state.
• Conduct virtual primary care appointments through telehealth, encompassing detailed assessments, management of chronic diseases, preventive screenings, and acute care interventions.
• Diagnose and oversee chronic conditions (such as hypertension, diabetes, COPD, and heart disease) while crafting personalized care plans to enhance patient health.
• Assess and address acute issues (including infections, respiratory ailments, minor injuries, and skin conditions) within the framework of virtual care.
• Order and analyze lab tests, imaging, and other diagnostic procedures, coordinating follow-up care as required.
• Manage medication prescriptions, adjustments, and refills while ensuring patient compliance and safety.
• Offer health coaching, lifestyle modification advice, and educational resources to empower patients in managing their health.
• Identify patients requiring referrals or home-based services and facilitate effective coordination with specialists or community resources.
• Utilize our electronic health platform and provide constructive feedback to support its continuous improvement, enhancing both patient and provider experiences.
• Engage in clinical case discussions, team meetings, and interdisciplinary collaborations to maintain high-quality, patient-centered care.
• Participate in ongoing learning and development, keeping current with best practices in virtual care and primary care management.
• Active Nurse Practitioner (APRN) or Active Physician Assistant (PA) license in good standing, with the capability to obtain licensure in multiple states within 90 days of hire.
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• At least 2 years of experience in primary care, urgent care, or internal medicine, preferably with a background in telehealth or virtual care environments.
• Current BLS certification at the time of hire.
• Shift times may vary to align with standard business hours (9:00 AM – 5:00 PM) in the Pacific time zone, requiring flexibility. Additionally, weekend shifts may become necessary based on evolving patient care demands.
