This is a fully remote position, open to applicants in Arizona, +3 more states.
• Conduct comprehensive virtual assessments of patients suffering from hypertension, hyperlipidemia, coronary artery disease, diabetes, and various other chronic illnesses.
• Develop diagnoses and personalized treatment strategies, incorporating medication management, lifestyle changes, and monitoring suggestions.
• Oversee prescription refill inboxes and manage asynchronous medication refills in accordance with established protocols and guidelines.
• Engage in centralized review, triage, and follow-up regarding laboratory and diagnostic findings.
• Utilize live portal chat, AI-assisted prescribing, and inbox triage tools effectively.
• Recognize risk factors, comorbidities, and potential treatment contraindications.
• Provide counseling to patients on medication adherence and possible side effects.
• Collaborate with primary care providers, specialists, pharmacy staff, clinical nursing, social work, family members, and caregivers.
• Employ the electronic health information system during virtual visits and give feedback on the interface.
• Ensure precise maintenance of patient medical records, adhering to legal and ethical standards.
• Engage in quality improvement initiatives and pursue professional development opportunities.
• Adhere to applicable laws, regulations, patient privacy mandates, and telehealth standards.
• Advanced Practice Provider: Nurse Practitioner or Physician Assistant.
• Minimum of 3 years of experience in outpatient clinical practice.
• At least 2 years focused experience in primary care or a relevant subspecialty, particularly with a substantial emphasis on hypertension and lipid management.
• Experience preferably includes hypertension, hyperlipidemia, and both primary and secondary prevention of atherosclerotic cardiovascular disease.
• Bilingual capability in Spanish and English.
• For Nurse Practitioners: active, in-good-standing RN and APRN licenses.
• For Physician Assistants: active, in-good-standing Physician Assistant license.
• Required role licensure and certification must be in good standing.
• Ability to acquire necessary licenses in requested states within 90 days of hire and obtain additional state licenses as required.
• Comprehensive understanding of cardiometabolic condition management, including disease processes, treatment options, medication management, and lifestyle modifications.
• Proficiency in English and Spanish is preferred.
• Familiarity with STARS/HEDIS measures, clinical comorbidity coding, and identifying care gaps.
• Comfortable utilizing digital portal chat and emerging AI-powered clinical tools.
• Virtual care experience is preferred.
• Experience or strong interest in centralized results management, digital/chat-based patient engagement, and AI-enhanced clinical workflows is a plus.
• Ability to work Monday through Friday in 8-hour shifts, typically from 8 a.m. to 5 p.m. MST/PST, with consideration for EST/CST hours.
