
Bilingual Spanish/English Active Primary Care APP – Telehealth

This is a fully remote position, open to applicants in Oregon, +1 more state.
• Deliver virtual primary care consultations via telehealth, encompassing thorough assessments, chronic disease management, preventive screenings, and acute care interventions.
• Diagnose and manage chronic illnesses (such as hypertension, diabetes, COPD, and heart disease) while creating personalized care plans to enhance patient health.
• Assess and treat acute issues (including infections, respiratory conditions, minor injuries, and skin disorders) within the parameters of virtual care.
• Order and analyze laboratory tests, imaging, and other diagnostic measures, coordinating follow-up care as needed.
• Oversee medication prescriptions, adjustments, and refills, ensuring adherence and safety for patients.
• Offer health coaching, lifestyle change advice, and patient education to empower individuals in managing their health effectively.
• Identify patients who require referrals or home-based services, facilitating appropriate coordination with specialists or community resources.
• Utilize and provide feedback on our electronic health platform, contributing to its ongoing enhancement to improve experiences for both patients and providers.
• Participate in clinical case reviews, team meetings, and interdisciplinary collaborations to ensure the delivery of high-quality, patient-centered care.
• Engage in continuous learning and development, remaining informed about best practices in virtual care and primary care management.
• Active Nurse Practitioner (APRN) license in good standing, with the ability to obtain licensure in multiple states within 90 days of hire.
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• Minimum of 2 years of experience in primary care, urgent care, or internal medicine, preferably with a background in telehealth or virtual care environments.
• Active BLS certification at the time of hire.
• Shift hours may vary to align with standard business hours (9:00 AM – 5:00 PM) in the patient's local time zone. Flexibility is essential. Additionally, weekend shifts may be required in the future based on the evolving needs of patient care.
