Bilingual, Spanish Speaking Medical Assistant

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform outbound telephonic outreach to patients and offer personalized coaching and support for those managing chronic conditions, including but not limited to type 2 diabetes, hypertension, and cardiovascular disease.

• Assist patients in implementing tailored care plans and reaching their objectives by providing education and coaching focused on behavior modification, nutrition, physical activity, and self-management strategies, utilizing evidence-based techniques such as motivational interviewing and SMART goals.

• Carry out thorough assessments of patients' health status, lifestyle choices, nutritional habits, and their readiness to change.

• Inform patients about disease management, medication adherence, symptom recognition, and preventive strategies, while performing care delivery tasks in line with Cadence clinical policies and established protocols, under suitable clinical supervision.

• Track patients' progress, compliance with treatment plans, and health outcomes through regular check-ins and remote monitoring tools.

• Record patient interactions, outcomes, and follow-up plans in the Cadence platform; identify barriers to care (including social needs) and connect patients with resources or internal support pathways.

• Collaborate in real-time with clinicians and patient support teams using platforms such as the Cadence Platform, Google Workspace, and Slack, and provide feedback to enhance workflows in a fast-paced, change-oriented environment.

⛳️ Requirements

• A valid Medical Assistant certification from an accredited association (either AAMA, AMCA, AMT, NHA, NAHP, or NCCT) is mandatory.

• A minimum of 5 years of experience as a Medical Assistant is necessary.

• Readiness for remote work (including reliable high-speed WiFi) is essential.

• Experience in internal medicine, cardiology, nephrology, or with the geriatric population is preferred.

• Familiarity with providing patient education and support for individuals managing chronic conditions is preferred.

• Proficiency in Spanish is required.

• Experience with AI-assisted clinical tools (such as Gemini, Open Evidence, or similar platforms) and the capability to communicate effectively with providers and patients regarding the role of AI in care delivery and the irreplaceable value of human judgment.
