Bilingual Spanish, RN Care Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Engage in telephonic care coordination and management for designated patient populations.

• Collaborate effectively with Clover's clinical team members.

• Initiate referrals to specialized services while promoting continuity of care.

• Develop individualized care plans tailored to each member.

• Record all member care activities and escalate relevant findings as necessary.

• Coordinate efforts with the interdisciplinary team and outpatient providers.

• Provide telephonic education and counseling focused on health promotion and disease management.

• Participate in regular ongoing meetings.

• Exhibit flexibility in assignments across Clover’s Clinical Programs.

• Advocate for members to ensure their needs and preferences are represented.

• Engage in continuous professional development.

⛳️ Requirements

• Minimum of 3 years of post-licensure experience in care management.

• Bachelor’s degree (BSN) or higher from an accredited nursing program.

• Current valid RN multi-state license.

• Proficiency in both English and Spanish, in verbal and written form.

• Case Management certification or intention to complete CCM certification within one year of employment (preferred).

• Experience in assessments, care plans, care coordination, and resolving issues.

• Knowledgeable about healthcare reimbursement, utilization management, discharge planning, and disease management.

• Background in geriatrics and chronic illness management.

• Basic understanding of Medicare and Medicare Advantage Plans.

• Comfortable with documenting in electronic medical records and using electronic data and reports.

• Intermediate computer skills on both Mac and PC platforms.
