Bilingual Spanish Nurse Case Manager – Care Transitions, RN, CA License

This is a fully remote position, open to applicants in California.

📋 Description

• Develop personalized discharge plans in collaboration with patients, their families, and the healthcare team

• Facilitate care coordination with healthcare providers, rehabilitation facilities, and home health agencies

• Track patient progress and modify plans as necessary

• Educate and empower patients and their families to manage their own care and follow-up

• Advocate for patient requirements and link them to community support resources

• Engage in care conferences, quality improvement projects, and interdisciplinary teamwork

⛳️ Requirements

• 2–3 years of experience in clinical care management (3–5 years preferred)

• Valid, unrestricted RN license in California (must be willing to obtain licenses for other states)

• Familiarity with Medicare Managed Care Plans, insurance regulations, and community resources

• Excellent communication, problem-solving, and organizational abilities
