Bilingual Spanish Health Coach, Educator

This is a fully remote position, open to applicants in California.

📋 Description

• Reach out to patients eligible for Alignment’s Care at Home program and offer essential wellness coaching aimed at reducing or eliminating high-risk behaviors.

• Work collaboratively with advanced practice clinicians and/or social workers to formulate an action plan.

• Promote the adoption of habits that enhance the quality of life.

• Accountable for delivering exceptional patient service and boosting member retention and satisfaction.

• Assist patients in accessing community resources and social services to mitigate social determinants affecting health and wellbeing.

• Create and distribute health education materials for identified patients, caregivers, and/or families.

• Coordinate care with other Alignment team members, including health-related vendors, to ensure optimal patient care.

⛳️ Requirements

• Required: At least 1+ year of experience as a wellness or health coach or educator.

• Preferred: Experience in senior care.

• Required: High School Diploma or GED; an Associate’s degree or two additional years of experience may substitute for education.

• Preferred: Bachelor’s Degree.

• Required: Ability to communicate positively, professionally, and effectively with others; demonstrate leadership, teach, and collaborate.

• Strong knowledge of specific diseases, lifestyle-related topics, nutrition, and exercise.

• Familiarity with and experience in applying stages of change or similar behavior change models.

• Excellent interpersonal skills to foster trust and respect among customers and coworkers.

• Capable of working independently and remotely.

• Competent in handling protected health information while maintaining confidentiality and privacy in line with HIPAA regulations and AHP policies.

• Proficient in computer skills, including Microsoft Office Excel, Word, and PowerPoint.

• Ability to work a flexible schedule.

• Strong written and oral communication skills; able to build and sustain constructive relationships with diverse members, management, employees, and vendors.

• Mathematical Skills: Ability to perform calculations and accurately compute simple statistics.

• Reasoning Skills: Ability to prioritize multiple tasks; advanced problem-solving skills; capable of using advanced reasoning to define problems, collect data, establish facts, draw valid conclusions, and design, implement, and manage appropriate solutions.

• Problem-Solving Skills: Strong problem-solving, organizational, and time management skills; ability to thrive in a fast-paced environment.

• Preferred: Bilingual in English and Spanish.
