This is a fully remote position, open to applicants in Arizona.
• Conduct in-depth and comprehensive assessments of patients with general cardiology and heart failure requirements via virtual consultations, which include ordering necessary diagnostics, interpreting laboratory and imaging results, and crafting a treatment plan in partnership with the specialty care clinic team.
• Develop precise diagnoses and create personalized treatment strategies for patients dealing with heart failure and general cardiology conditions, encompassing medication management, volume monitoring, and lifestyle changes.
• Initiate, adjust, and oversee guideline-directed medical therapies (GDMT) for a varied group of members experiencing heart failure.
• Deliver proactive management of heart failure and associated comorbid conditions, addressing potential obstacles to optimal care to reduce the risks of heart failure exacerbations and hospitalizations.
• Evaluate and intervene in cases of weight fluctuations and acute volume overload management for patients enrolled in the clinic.
• Assess, diagnose, and manage a range of general cardiology conditions that are appropriate for virtual care, including stable coronary artery disease, hypertension, hyperlipidemia, stable atrial arrhythmias, peripheral arterial disease, and chronic venous disease.
• Collaborate closely with other care team members, including PCPs, cardiologists, and other professionals such as pharmacy, clinical nursing, and social work, as well as engaging with family members and caregivers to coordinate comprehensive care for the member, ensuring continuity and delivering a collaborative care plan.
• Keep precise and current patient medical records, ensuring adherence to relevant legal and ethical standards.
• Engage in quality improvement initiatives and continuous professional development to remain updated on best practices and advancements in cardiovascular care.
• NP or PA with 5 or more years of experience in cardiology clinical practice, ideally with 2 years in an outpatient cardiology setting.
• Bilingual in English and Spanish.
• Experience conducting visits via telehealth video platforms and a strong desire to continue practicing virtually.
• A secure and private workspace equipped with a strong and reliable internet connection.
• Highly proficient with technology (EMRs, messaging platforms, online portals, AI chat).
• Willingness to work from 8 AM to 5 PM in the Central Time Zone or later time zones (MT, PCT).
• Willingness to obtain and maintain multistate licensure within the first 90 days (support and reimbursement provided).
• An understanding of managed care is advantageous, including the ability to appropriately assess STARS/HEDIS measures, code clinical comorbidities, and identify clinical care gaps.
