
Bilingual RN Care Manager – Spanish Speaking

This is a fully remote position, open to applicants in California.
• Primarily engage with and support a caseload of individuals who have complex medical requirements.
• Collaborate with individuals to define health and wellness objectives, integrating these goals into a Shared Care Plan.
• Educate individuals about medical and behavioral health issues (including medications) to enhance their health literacy.
• Conduct medication reconciliation in partnership with the individual’s pharmacy.
• Offer care management services, which include coordinating prescriptions and managing prior authorizations.
• Monitor and ensure that all necessary assessments and screenings are completed.
• Work together with a multidisciplinary care team to identify and overcome barriers to care.
• Recognize clinical needs and triage escalations, providing brief interventions as needed, with support from nurse practitioner clinicians.
• Collaborate on care matters with the Enhanced Care Management team by engaging in systematic case reviews.
• Consult with the Enhanced Care Management team regarding clinical concerns or inquiries; provide educational training on chronic disease management, prevention, treatment, medications, and healthy living.
• Establish trust and foster relationships with individuals facing homelessness, those living with Severe Mental Illness/Substance Use Disorder, and individuals with multiple chronic conditions.
• Utilize relationship-based strategies to engage individuals in their care, recognizing that many may have personal experiences that lead to initial hesitance or distrust of the healthcare system.
• Strive to listen openly to individuals and meet them where they are, embracing an “it’s not my fault but it is my problem” mindset in all forms of communication and approach.
• Must possess an active Registered Nurse license issued by the state of California.
• Must be located in California (preferred).
• Prior experience in care coordination or case management is required.
• 2-3+ years of experience working with a health plan or at-risk provider.
• Bilingual in English and Spanish.
• Strong technical skills and comfort with new technological innovations, including prior experience with CRM databases, basic Google Suite, email, and video conferencing.
• Must have a quiet, HIPAA-compliant work environment at home, with a reliable Internet connection and cell phone.
• A solid understanding of cultural fluency is essential.
• Demonstrated professional or personal lived experience closely working with individuals facing complex chronic needs, homelessness, or Severe Mental Illness/Substance Use Disorder is necessary.
