This is a fully remote position, open to applicants in United States.
• Deliver proactive and professional care while effectively triaging clinical concerns.
• Incorporate a family-centered care approach in the delivery of services.
• Exhibit cultural competence and sensitivity when engaging with diverse populations.
• Manage transitions of care for ED/IP/UC services in collaboration with clinical providers post-discharge.
• Conduct thorough assessments addressing clinical, psychosocial, discharge planning, and financial requirements.
• Build rapport with patients and their families to understand their needs and expectations and establish realistic shared goals.
• Develop comprehensive care plans in cooperation with physicians, patients, and the interdisciplinary team.
• Communicate the care plans and any updates to patients, families, physicians, and relevant healthcare team members.
• Collect data to assess the effectiveness of care plans and ensure that care is safe, timely, accurate, and cost-efficient.
• Document care management assessments and interventions appropriately.
• Refer complex psychosocial and discharge planning matters to Social Work or Behavioral Health, coordinating necessary follow-ups.
• Consult with interdisciplinary team members, including dietary and pharmacy, to facilitate safe discharge plans.
• Perform additional tasks as assigned.
• Must hold a valid RN license in at least one state, with the ability to apply for licensure in other states.
• A Bachelor’s degree in nursing from an accredited institution is required.
• A minimum of 5 years of experience in pediatrics is required.
• Preferably, at least 1 year of experience in care coordination or case management.
• Bilingual in Spanish is a requirement.
• Familiarity with Medicaid regulations and services is advantageous.
• Experience in Value-Based Care (VBC) is a plus.
• Virtual care experience is considered a plus.
• Must be physically present in the United States.
• Availability to work the standard 3x12 schedule: Monday-Wednesday or Wednesday-Friday, from 7:00 am to 7:00 pm CST.
