Bilingual Patient Navigation – Non-Clinical

This is a fully remote position, open to applicants in Alabama.

📋 Description

• Establish connections with patients through phone outreach following a clinical event or diagnosis.

• Collaborate with the clinical team and utilize health-system resources to enhance the patient experience.

• Investigate, identify, and evaluate community resources available for patients.

• Engage in a professional network that sets care-guidance standards based on disease states.

• Enable patients to express their preferences and priorities to healthcare teams, facilitating their involvement in healthcare decisions.

• Assist in the development, execution, and assessment of the Care Guide navigation program.

• Tackle non-clinical factors that influence patient health.

• Perform additional responsibilities that align with company values, mission, and policies.

• Meet productivity targets for high-quality and timely client delivery.

• Ensure consistent and dependable attendance during scheduled hours.

• Aid patients and caregivers in overcoming practical, social, physical, and emotional challenges.

⛳️ Requirements

• Proficient with personal computers.

• Possess a technical and analytical skill set.

• Exceptional communication, organizational, and interpersonal abilities.

• Experience with Microsoft Excel is preferred.

• A bachelor's degree is preferred; equivalent professional experience will be taken into account.

• Bilingual in Spanish and English.

• Capable of working remotely in a private HIPAA-compliant workspace.

• Must have the capacity to accommodate company equipment required for the role.

• Access to broadband internet.

• Internet download speed of at least 24 Mbps and upload speed of at least 4 Mbps.

• Ability to hear within the normal range and use a headset/earpiece.

• Good visual acuity for reading computer screens, scripts, and forms.

• Capability to sit for up to 100% of the time while taking calls.

• May need to utilize a personal mobile device for EMR authentication.

• Must successfully pass a background check, drug screening, and identity verification process.

• Must be authorized to work without requiring immigration or work visa sponsorship.

• Regular and reliable attendance during scheduled hours is necessary.
