This is a fully remote position, open to applicants in Canada.
• Provide assistance to patients and healthcare providers by delivering information regarding Patient Support services.
• Support patients and caregivers in performing assessments related to Patient Support services.
• Serve as a liaison between patients, families, and healthcare providers concerning medication access.
• Respond to general inquiries about program services, disease states, or product-related questions.
• Provide ongoing assistance regarding disease states or product-related inquiries to promote patient adherence.
• Refer patients to their healthcare providers for clinical inquiries outside the scope of program materials.
• Record patient information and interactions within the electronic patient record system.
• Conduct regular file reviews and updates to accurately reflect the patient journey.
• Adhere to case-management protocols, including data collection, patient follow-up, monitoring therapy disruptions/discontinuations, and reporting adverse events.
• Collaborate with the reimbursement specialist and physician to secure funding for patient therapies.
• Ensure coordination and tracking of patient services in alignment with KPIs.
• Participate in specialized training sessions for specific therapies offered by the pharmaceutical team.
• Attend pharmaceutical meetings, patient meetings, conferences, and collaborate with patient associations and advocacy groups as necessary.
• Coordinate with healthcare providers, pharmacies, and infusion/injection clinics to manage treatment logistics and resolve issues.
• Maintain open communication with the manager, team, and client regarding the management of patient cases.
• Assist in the onboarding of new Nurse Case Managers and provide feedback on protocols.
• A Diploma in healthcare or a related field, with a preference for a Bachelor’s degree.
• Licensed Nurse (RN or RPN/LPN) with active registration from the College of Nurses in the province of employment.
• A minimum of two (2) to five (5) years of experience in healthcare, pharmaceutical, insurance, or related fields.
• Ideal candidates should be nurses with a background in healthcare and case management within medical/hospital settings, insurance, or the bio-pharmaceutical industry.
• Fluency in French and/or English may be required based on the program and geographic location.
