Bilingual Nurse Case Manager – 12-Month Contract

This is a fully remote position, open to applicants in Canada.

📋 Description

• Serve as the main liaison for prescribers and clinical teams in enrolling patients and coordinating services.

• Deliver patient education regarding disease information, medications, and side-effect management, along with injection training for home, clinic, or virtual administration based on patient preferences.

• Foster treatment adherence through scheduled follow-ups and efficiently relay clinical updates to physicians and clinical staff.

• Ensure precise and thorough documentation while conducting chart reviews as necessary to facilitate reimbursement.

⛳️ Requirements

• Bilingual: Proficient in both French and English, in spoken and written forms.

• A valid nursing license in Ontario or Quebec (or both).

• Registered Nurse (RN) designation is highly preferred; Registered Practical Nurses (RPN), Licensed Practical Nurses (LPN), and Nurse Practitioners (NP) will also be considered.

• At least 2 years of pertinent experience (or an equivalent combination of education and experience).
