Bilingual LPN, Personal Care Coordinator I – Spanish

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct monthly care management discussions via outbound calls to designated patients to address their healthcare requirements.

• Share information about resources and services accessible to patients throughout the continuum of care.

• Recognize patient-specific challenges, objectives, and interventions tailored to fulfill the individual needs of patients through action-oriented and timely goals.

• Ensure chart compliance and accurate documentation, including but not limited to medical history, medications, immunizations, allergies, surgical history, and family history.

• Exhibit awareness of a patient’s situation that may require adjustments to care plans, goals, and preferences, while being cognizant of obstacles to care and services.

• Offer suitable health education.

• Forward patient concerns to the triage nursing team.

• Active, compact LPN/LVN license.

• Bilingual and fluent in Spanish.

• Strong capability to adhere to established clinical protocols and procedures.

• Ability to adapt and switch tasks effectively.

• Willingness to support colleagues and contribute to team success.
