This is a fully remote position, open to applicants in United States.
• Conduct phone consultations with patients participating in care management and/or remote patient monitoring programs, offering support and education regarding their chronic conditions.
• Instruct patients on their health conditions and equip them with lifestyle and behavioral strategies to take an active role in managing their chronic conditions.
• Help patients establish and achieve goals that align with their provider-approved care plans.
• Keep accurate and current patient records, ensuring that all interactions and care plans are documented according to established protocols.
• Address patient concerns and obstacles to care, striving to identify practical solutions that enhance patient adherence and outcomes.
• Deliver clear, compassionate, and effective communication to patients.
• Adhere to approved workflows for communicating patient needs to their healthcare providers.
• Engage in training sessions, team meetings, and quality improvement initiatives to improve the care navigation process and enhance the patient experience.
• Respond to remotely transmitted patient data, such as blood pressure, blood glucose, weight, and pulse oximetry, in accordance with approved partner workflows.
• A current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN).
• Over 3 years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring.
• Strong communication and interpersonal skills.
• Proficiency in verbal and written communication in Spanish.
• Exceptional organizational and time management skills.
• Competence in using electronic health records (EHR) and care management software.
• Ability to work autonomously and collaboratively within a team.
• Empathetic and patient-centered approach to care.
• Reliable internet connection, a HIPAA-compliant workspace, and proficiency with virtual communication tools (e.g., Zoom, Slack).
