Bilingual Clinical Nurse Coach, RN

This is a fully remote position, open to applicants in Arizona.

📋 Description

• In the role of Clinical Nurse Coach, you will provide remote health coaching to members who are at risk for chronic kidney disease or who already have the condition.

• You will collaborate with our members to enhance their understanding and management of conditions and behaviors that affect kidney health, empowering them to take control of their health and avoid dialysis.

• You will establish and maintain communication with assigned and active members through video, phone, text, and email in both English and Spanish.

• You will gather and apply clinical information to create a tailored member care plan and clinician-centered care plan following NCQA guidelines.

• You will evaluate members' health status and care coordination requirements.

• You will collaborate with members’ healthcare providers as necessary.

• You will employ motivational interviewing techniques, personalized care plans, and the Triple Aim framework to motivate active members in achieving their health objectives.

• You will explain to new or inactive members the advantages of our Program related to chronic kidney disease and those at risk.

• You will ensure proper documentation, requests, and transmission of member information in compliance with HIPAA regulations.

• You will meet individual performance metrics to enhance member engagement and retention.

⛳️ Requirements

• A valid RN license in good standing is required, along with a willingness to obtain licensure in other states.

• A minimum of five years of RN experience is mandatory.

• Candidates must reside in PST or MST and work at least three late shifts per week.

• Experience in developing care plans for individuals with chronic conditions and/or Chronic Kidney Disease is strongly preferred.

• Fluency in speaking, reading, and writing in both English and Spanish is essential.

• A BSN is highly preferred.

• CDE or CDCES certification is highly preferred.

• Experience working 100% remotely as a nurse is highly preferred.

• Willingness and ability to travel occasionally, which may include overnight stays for corporate events, conferences, and health fairs.

• Ability to participate and engage professionally in video meetings.

• Proficient technological skills are required, including efficient use of computers, peripheral devices, and software systems, particularly Microsoft products.

• An autonomous self-starter who is comfortable with ambiguity is essential.

• A creative mindset with the capacity to appropriately challenge the status quo is important.

• Excellent written and verbal communication skills are required.

• A proactive approach to overcoming obstacles with a ‘yes if...’ mindset is necessary.

• Ability to effectively manage competing deadlines while maintaining focus on the overall objectives is critical.

• A reliable internet connection and power supply, along with a designated workspace that minimizes interruptions, are required.
