Bilingual Care Coordinator – LVN, Illinois License

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Execute monthly Chronic Care Management (CCM) calls to evaluate patient health status and deliver patient education.

• Conduct medication reconciliations and identify obstacles to adherence in care.

• Aid in health risk assessments, remote patient monitoring, and transitional care management initiatives.

• Precisely document all patient interactions within Electronic Health Record (EHR) systems.

• Collaborate with healthcare providers and clinic personnel to facilitate personalized patient care plans.

• Connect patients with suitable community resources and support services.

• Uphold HIPAA compliance and ensure patient confidentiality at all times.

• Independently oversee schedules, patient outreach, and productivity targets.

⛳️ Requirements

• Valid, unrestricted Illinois LVN/LPN license.

• Bilingual proficiency in English and Spanish.

• Current Basic Life Support (BLS) certification.

• Strong capability to work autonomously and manage time efficiently.

• Exceptional communication and critical thinking abilities.

• High level of proficiency with Electronic Health Records (EHR) systems and Google Suite.

• Dedicated HIPAA-compliant home office area with a private, secure environment and a locked door.

• Reliable high-speed internet connection.

• Prior experience with Chronic Care Management (CCM) programs and billing workflows (Nice to Have).

• Experience in remote healthcare, telehealth, or virtual patient support settings (Nice to Have).

• Familiarity with Atlas or comparable clinical management software platforms (Nice to Have).
