Bilingual Care Coordinator I

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct monthly CCM phone calls with the designated practice.

• Make 30–50 outbound calls each day.

• Complete necessary patient interactions to achieve CCM service objectives.

• Educate patients about chronic conditions and their care plans.

• Coordinate care with healthcare providers, pharmacies, and community resources.

• Identify gaps in care as well as social determinants affecting health.

• Create and update personalized care plans within the EMR system.

• Accurately document all interactions with patients.

• Ensure HIPAA compliance and maintain patient confidentiality.

⛳️ Requirements

• A minimum of 1-3 years of experience as a CMA/RMA, LPN, or RN.

• Active certification or multistate license for LPN/RN.

• Exceptional verbal and written communication skills in English.

• Comfortable making phone calls for most of the workday.

• Familiarity with EMR/EHR systems.

• Organized, dependable, and capable of working independently in a remote environment.

• Preferred: Experience with geriatric patients.

• Preferred: Experience in care coordination or case management.

• Strong verbal and written communication skills in Spanish.
