This is a fully remote position, open to applicants in Colombia.
• Serve as the primary communication link among patients, physicians, specialists, and multidisciplinary care teams.
• Assist in the creation, updating, and monitoring of personalized patient care plans.
• Monitor progress towards clinical objectives and arrange necessary follow-up appointments.
• Ensure precise maintenance of electronic health records (EHR/EMR).
• Manage referrals and conduct insurance eligibility verification.
• Address incoming patient inquiries through phone, email, or chat.
• Connect patients with community resources, home care services, medical equipment providers, and financial assistance programs.
• Perform routine patient check-ins to assess treatment adherence and tackle any barriers to care.
• Provide education to patients regarding their prescribed care plans.
• Ensure that coordination activities comply with HIPAA regulations, patient privacy laws, and standard operating procedures (SOPs).
• Identify treatment delays, care gaps, and recurring operational challenges.
• Elevate issues to clinical leadership for resolution.
• Over 3 years of experience in healthcare coordination, medical administrative support, patient navigation, or case management.
• Proficiency in Electronic Health Record (EHR) systems.
• Proficiency in HIPAA-compliant communication tools.
• Proficiency in Microsoft Office Suite and Google Workspace.
• Excellent verbal and written communication skills.

• Capacity to manage multiple care tracks concurrently.

• Previous experience in remote healthcare or telehealth is preferred.
• Knowledge of medical billing, coding, or insurance verification is preferred.
