This is a fully remote position, open to applicants in Oregon.
• Conducting in-person assessments and developing person-centered service plans in collaboration with individuals and their assigned representatives.
• Ongoing evaluation of service plans.
• Supporting transitions between community and institutional environments.
• A minimum of 2 years of clinical experience, which includes performing assessments, collecting histories, conducting examinations, providing DSM 5 diagnoses, and formulating treatment and safety plans for individuals with severe and persistent mental illness (SPMI).
• Must be a Qualified Mental Health Professional (QMHP) or provide evidence of a QMHP application upon receiving an offer, with certification to be acquired within 2 weeks of employment; alternatively, possess current, active, unrestricted clinical licensure in the state of Oregon.
• Applicants should reside within the geographical area and be prepared to travel in their personal vehicle up to 25% of the time for in-person assessments.
