Behavioral Health Care Manager – BHCM

This is a fully remote position, open to applicants in California, +8 more states.

📋 Description

• Allocate approximately 75% of your time engaging with patients and their families.

• Provide brief, evidence-based interventions aimed at fostering significant change within 6–12 months.

• Work closely with pediatricians, psychiatrists, and a multidisciplinary care team.

• Emphasize structured, impactful care rather than open-ended therapy.

• Operate within evidence-based clinical pathways focused on achieving outcomes.

• Join a team-oriented model where you will receive support from: Pediatricians, Psychiatric consultants, and Care coordinators.

• Offer integrated care that encompasses: Behavioral health interventions, Care coordination (school, SDoH, therapies), and Family education and support.

• Conduct intake assessments and ongoing follow-up sessions using telehealth.

• Implement brief, evidence-based interventions such as: CBT skills, Motivational interviewing, Behavioral activation, and Parent coaching.

• Provide in-session education and actionable recommendations to families.

• Monitor patient progress utilizing validated screening tools.

• Assist families with: School accommodations, SDoH needs, and Coordination with OT/PT/ST and other specialty services.

• Collaborate with pediatricians and psychiatric consultants.

• Engage in weekly registry review meetings to inform care decisions.

• Partner with board-certified pediatric psychiatrists who: Offer medication recommendations, and Oversee patient progress and care plans.

• Document all care activities in our EHR (Healthie).

• Track outcomes and progress through structured workflows.

• Manage scheduling, follow-ups, and patient engagement.

• Utilize standardized protocols for no-shows, cancellations, and safety.

⛳️ Requirements

• Possession of an active, unrestricted clinical license as a licensed clinical social worker (LCSW), licensed professional counselor (LPC), and/or licensed therapist (LMFT).

• Active licensure in one or more of the following states by the start date: California, Colorado, Connecticut, Florida, Maryland, Tennessee, Texas, Virginia, or Washington.

• Experience working with children, adolescents, and families.

• A minimum of 3 years of experience OR at least 3,000 supervised hours.

• Flexibility in working hours/availability, including 2 evenings per week and 2-4 weekend days per month.

• Familiarity and training in evidence-based modalities (CBT, MI, BA, etc.).

• Comfort with a telehealth environment.

• Strong skills in communication, organization, and documentation.

• Ability to provide care through a diversity, equity, and inclusion (DEI) and trauma-informed perspective.
