Ambulatory Care Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Collaborate with designated patients and/or beneficiaries to evaluate, facilitate, plan, and advocate for their unique health requirements.

• Aid in organizing cost-effective healthcare services.

• Develop transition plans for post-acute care.

• Assist in managing transitions between different levels of care through effective care coordination.

• Serve as a liaison among patients, consulting and referring physicians, and external agencies to arrange appointments, admissions, or surgeries.

• Organize both written and verbal communication before and after patient consultations.

• Work in partnership with healthcare team members to enhance patient care.

• Complete psychosocial assessments and evaluate family dynamics involving patients and their families.

• Review electronic records and charts to gain insights into patient history and current situations.

• Evaluate care plans to ensure they align with patient needs.

• Provide education to families, patients, and healthcare team members.

• Achieve organizational objectives that resonate with Franciscan Values and Service Standards.

⛳️ Requirements

• Associate's Degree in Nursing/Patient Care — Required.

• Bachelor's Degree in Nursing/Patient Care — Preferred.

• Indiana Registered Nurse (RN) state license — Required.

• Illinois Registered Nurse (RN) state license must be obtained within 180 days of employment.

• Certified Case Manager (CCM) — Preferred.

• Basic Life Support (BLS) certification from the American Heart Association — Required.

• Minimum of 5 years of nursing/patient care experience — Required.

• At least 1 year of case management experience — Preferred.

• Weekend position: Saturday and Sunday, with 10-hour shifts.

• Up to 20% travel may be required.
