This is a fully remote position, open to applicants in Alabama, +11 more states.
• Conduct initial evaluations and create Palliative Care Plans for members who are newly enrolling in the Palliative Care Program.
• Manage a longitudinal panel of high-risk patients (approximately 40-50) through regular telephonic and/or video interactions.
• Provide consultative visits for patients assigned to the RNs, encompassing advanced symptom management, complex goals of care, advanced disease education, prognostication, and medical orders for home health, DME, medications, and hospice transitions.
• Assess hospital admissions for their underlying causes; aid in transitions of care for engaged patients, pinpoint trends in readmissions, and contribute to developing solutions to maintain a readmission rate of 15% or less.
• Participate in regular huddles with clinical and operations team members.
• Attend weekly interprofessional team meetings.
• Conduct monthly panel reviews with your Medical Director and Operations Manager.
• Host a 30-minute office hours session each week.
• Provide support to the team during periods of rapid growth or employee absences; this may include handling phone lines, making outreach calls, conducting initial visits, and monitoring incoming consults.
• Collaborate with interdepartmental clinical teams and leadership to ensure seamless care for shared patients.
• Attend Devoted Medical meetings as required for your role, which may include All-Hands, Clinical Excellence, and Orinoco Updates meetings.
• Provide on-call coverage during weekends and holidays on a rotating schedule.
• Uphold the Devoted Health mission by treating members as family.
• An unrestricted advanced practice license in one or more of the Devoted Membership states: FL, TX, AZ, OH, CO, OR, TN, IL, AL, HI, NC, SC, PA.
• Willingness to obtain additional state licenses within 90 days of request.
• A minimum of 5 years in advanced practice, including at least 3 years in Palliative care.
• ACHPN® credential, or the ability and willingness to obtain it within the first six months of employment.
• Flexibility to adjust between time zones.
• Proficient with technology and accustomed to a fast-paced work environment.
• Self-motivated individual capable of succeeding in a remote work setting.
• Excellent interpersonal and communication abilities.
• Comfortable working within an interprofessional care team.
• ELNEC® or EPEC® training (Preferred).
• Previous experience in ICU, oncology, or geriatrics nursing (Preferred).
• Experience in home-based palliative care (Preferred).
• Prior leadership experience within an interprofessional team (Preferred).
• Background in program development (Preferred).
