This is a fully remote position, open to applicants in Utah.
• Perform thorough intake assessments for new patients, recognizing unmet needs, objectives, and constraints.
• Create and facilitate personalized care plans in partnership with a multidisciplinary care team.
• Educate patients and referring clinicians regarding survivorship-oriented care.
• Assist and mentor interdisciplinary team members in formulating patient treatment strategies.
• Liaise with patients’ oncology teams and primary care providers as necessary.
• Provide empathetic, patient-focused care through a secure telehealth platform.
• Valid licensure as a Physician Associate or Nurse Practitioner in the U.S.
• Minimum of five years of experience in oncology or palliative care (preferred).
• Capability to work autonomously and solve problems in a fast-paced virtual environment.
• Excellent communication skills and a strong ability to build relationships with patients and colleagues.
• Deeply compassionate, patient-focused approach to care delivery.
• A private, HIPAA-compliant home office for telehealth consultations.
• High-performance personal computer equipped with a webcam, dependable high-speed internet, and a smartphone.
• Proficiency in using cloud-based tools such as G-Suite, electronic medical records, and VOIP systems.
• Comprehensive understanding of HIPAA, HITECH, and patient confidentiality standards.
• Structured onboarding and training process.
