Admissions Care Coordinator

This is a fully remote position, open to applicants in Florida, +3 more states.

📋 Description

• Oversee the onboarding process for patients from referral to the initiation of therapy, ensuring a smooth and timely experience.

• Confirm insurance coverage and relay financial responsibilities to patients.

• Identify, secure, and manage financial assistance and copayment support.

• Ensure accurate and comprehensive patient records are maintained throughout the course of care.

• Collaborate with cross-functional teams to effectively implement care plans.

• Act as a liaison for providers, payers, and referral sources.

• Track workflows and pinpoint opportunities for enhancing efficiency and patient satisfaction.

• Assist in onboarding and mentoring new team members.

• Maintain and improve policies and procedures.

• Address and resolve any patient or partner issues promptly.

⛳️ Requirements

• High School Diploma or GED required.

• At least 1 year of experience in healthcare, customer service, contact center, or a similar field.

• Excellent communication skills.

• Proficient in Microsoft Office.

• Capable of managing several priorities simultaneously.

• Strong organizational and problem-solving abilities.

• Dedicated to teamwork and enhancing the patient experience.

• Preferred: Experience in pharmacy, home health, or home infusion.

• Preferred: Familiarity with medical terminology and insurance verification processes.

• Preferred: Background in supporting financial assistance programs.

• Preferred: Previous mentoring experience.
