Adherence Care Navigator 2, CPhT

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Make 80–100 structured outbound calls daily to Medicare members, pharmacies, and healthcare providers.

• Identify and address medication adherence challenges for patients with chronic illnesses such as diabetes, hypertension, hyperlipidemia, and asthma.

• Adhere to established call guides.

• Deliver patient education within the specified scope.

• Collaborate with pharmacies and healthcare providers as necessary.

• Record all interactions in real time across various systems.

• Uphold quality and professionalism in a metrics-oriented, high-volume call center setting.

⛳️ Requirements

• Valid CPhT certification from PTCB and national certification.

• High School Diploma or GED.

• At least 2 years of experience in pharmacy or related patient interaction roles.

• 2–5 years of experience with pharmacy operations and substantial outbound calling.

• Proficiency in Microsoft Word, Excel, Outlook, and Google applications.

• Capability to learn new technologies.

• Experience with Medicare/managed care is preferred.

• Ability to work independently in a remote setting.

• Must be fluent in English and a second language if bilingual; proficiency in Spanish, Mandarin, Korean, Cantonese, or Vietnamese is advantageous.

• Availability to work Monday–Friday, 40 hours per week, during an 8:00 AM–8:00 PM CST shift range.

• Must be able to hardwire directly to a router/modem; T-Mobile, satellite, and 5G internet connections are not acceptable.

• Internet speed must be at least 50 Mbps download and 10 Mbps upload.

• A quiet, distraction-free workspace is required.
