Adherence Care Navigator 2 – CPhT

This is a fully remote position, open to applicants in Kentucky.

📋 Description

• Make 80–100 structured outbound calls each shift/day to Medicare members, pharmacies, and healthcare providers.

• Identify and address medication adherence challenges for patients with chronic conditions such as diabetes, hypertension, hyperlipidemia, and asthma.

• Follow established call protocols.

• Deliver patient education within the defined scope of practice.

• Collaborate with pharmacies and healthcare providers as necessary.

• Record all interactions in real-time across various systems.

• Uphold quality, professionalism, empathy, and active listening within a metrics-driven environment.

• Adhere to HIPAA regulations and ensure documentation accuracy.

⛳️ Requirements

• Current CPhT certification through PTCB; nationally recognized certification.

• High School Diploma or GED equivalent.

• At least 2 years of experience in pharmacy or a related field involving patient interaction.

• 2–5 years of experience in pharmacy with substantial outbound calling.

• Experience in making medication adherence calls.

• Proficiency in Microsoft Word, Excel, Outlook, and Google applications.

• Capability to learn new technologies.

• Ability to work independently in a remote setting.

• Must hardwire directly into a router/modem; T-Mobile, satellite, or 5G internet is not permitted.

• Internet connection with a minimum speed of 50 Mbps for downloads and 10 Mbps for uploads.

• A quiet, distraction-free workspace for remote work.

• Proficient in English; bilingual proficiency in Spanish, Mandarin, Korean, Cantonese, or Vietnamese is advantageous.

• Experience with Medicare/managed care is preferred.

• Availability to work Monday–Friday, 40 hours per week, during the shift range of 8:00 AM–8:00 PM CST.
