Adherence Care Navigator 2, CPhT

This is a fully remote position, open to applicants in Florida.

📋 Description

• Perform 80–100 structured outbound calls each shift/day within a fast-paced remote call-center setting.

• Reach out to Medicare members, pharmacies, and healthcare providers.

• Discover and address medication adherence challenges for patients with chronic illnesses including diabetes, hypertension, hyperlipidemia, and asthma.

• Adhere to structured call protocols.

• Offer patient education within established guidelines.

• Collaborate with pharmacies and healthcare providers as necessary.

• Accurately document all interactions in real-time across various systems.

• Uphold quality, professionalism, and efficiency in a metrics-focused environment.

• Operate autonomously in a remote work setting.

⛳️ Requirements

• Valid CPhT certification from PTCB and national certification.

• High School Diploma or GED.

• At least 2 years of experience in pharmacy or related patient interaction roles.

• 2–5 years of experience in pharmacy with significant outbound calling.

• Background in making medication adherence calls.

• Proficient in Microsoft Word, Excel, Outlook, and Google applications.

• Capability to learn new technologies.

• Medicare/managed care experience is preferred.

• Ability to work independently in a remote setting.

• Must be fluent in English, and a second language if bilingual.

• Availability to work 40 hours per week, Monday through Friday.

• Must be able to establish a direct hardwired connection to a router/modem; T-Mobile, satellite, and 5G internet are not permitted.

• Internet speed must meet a minimum of 50 Mbps download and 10 Mbps upload.

• A quiet, distraction-free workspace is required for remote work.
