Adherence Care Navigator 2 – CPhT

This is a fully remote position, open to applicants in Alabama.

📋 Description

• Perform 80–100 structured outbound calls daily during each shift in a high-volume remote call center.

• Reach out to Medicare members, pharmacies, and healthcare professionals.

• Detect and address medication adherence challenges for patients suffering from chronic conditions such as diabetes, hypertension, hyperlipidemia, and asthma.

• Adhere to structured call guidelines and deliver patient education within established parameters.

• Collaborate with pharmacies and healthcare providers as necessary.

• Record all interactions in real-time across various systems.

• Uphold quality, professionalism, and performance standards in a metrics-driven setting.

⛳️ Requirements

• Current CPhT certification from PTCB and national certification.

• High School Diploma or GED required.

• At least 2 years of experience in pharmacy or related patient interaction roles.

• 2–5 years of experience with pharmacy practices and intensive outbound calling.

• Proficient in Microsoft Word, Excel, Outlook, and Google; capable of adapting to new technology.

• Experience with Medicare or managed care is preferred.

• Ability to work autonomously in a remote setting.

• Must be fluent in English, with a second language preferred; Spanish, Mandarin, Korean, Cantonese, and Vietnamese are advantageous.

• Availability to work 40 hours a week, Monday–Friday, within the 8:00 AM–8:00 PM CST shift window.

• Must connect directly to a router/modem; T-Mobile, satellite, and 5G internet connections are not acceptable.

• Internet speed requirement of at least 50 Mbps download and 10 Mbps upload.

• A quiet, distraction-free workspace is essential.

• Ability to fulfill all work-from-home prerequisites.
